The overload spring looks like it is closer to the bottom leaf on the right side.

They look like they are frowning pretty good. I would look into some new springs. Hop in the box and jump. If the springs bounce the bed back pretty quick, then you should be alright for a while. If they dont bounce back very quickly than thats a sign of fatigue.

Could also be a slightly bent frame. Has the truck seen any really heavy loads? Heavy loads and a rough road will do a number on our stock springs. My frame is bent a little on the one side

I tried to reply last night but my interwebs decided to crap out. I agree with MapleMoose. Those springs have probably served their life on that truck. Look into some new stock springs or lift it with a new leaf pack in the back. Your talking about springs that have been on the road for 13 years.

thats true, just did the jump test and they took about .5 seconds. Is that slower than it should be?

Dude buy some springs. They look like hell and its a good thing to upgrade. Are things pretty level up front?

If the front is truly level and the rear is that twisted something is bent. Semi common items are bent frame, failing cab mount bushings and and the cab itself.
Think about it. Take a rectangle of cardboard, this is your frame. Hold one end level and ask yourself what has to happen to make the other end twist (lean).
Now if one of the rear springs was cracked or completely shot, there is a fair bit of flex in the frame that could account for "some" lean. Yours looks pretty bad.
Look for an uneven gap between cab and bed. Stick your finger in the gap and run it up and down. The gap should be constant.

I'd bet you money that your right leaves are sagging. It sure looks like it.

    Simple.

My 01 PreRunner is suffering from the same problem, only on the left side tho. It has 165k miles so I suppose it's time for an upgrade. People have suggested to me Billstein 5100's but I'm questioning them because of the raised ride height. Have the original rims and tires and I think it would look weird with the lift. Anyone know if I can get the shocks and springs and stuff but not have the increased ride height?

Taco lean. Heavy shit is mounted on driver side. 5100's have stock setting. I'm on 3" of lift on stock wheels n tires. Looks pretty good still. Don't be afraid to lift :D
