Here's the link and the new instructions show how they assemble on that same (revised) page. The top nut should also be torqued to 150 ft-lbs. The old instructions were wrong stating 85 ft-lbs. Hope you can get it sorted out quick and easy with the pics and instructions with them.

 No problem bud. That's what we're all here for. Good luck with everything.

Then that's why they are spinning. You have to have that flat (with the star pattern in the middle) washer to hold it in place and keep it in the grove of the UCA.

Just a quick update...I dropped the truck off yesterday morning, they worked on it again and when I called last night they needed to order some bushings. I assume for the LCA, but I don't know. They were brief on the phone and I was at work. The point is, no Taco yet. Still in the shop..

They did say the guy who was doing the alignment said the truck was scary to drive so that's a little validation that I wasn't crazy.
This is day six without the truck though, including the weekend, and I'm starting to lose my mind...

Wait a minute. Aren't the control arms installed on the wrong side. I thought the ball joint should be on the right side of that pic toward the rear of the truck.

Or do you have the "front of truck", "rear of truck" marked incorrectly?

They said it was scary to drive but they let you have it back anyway? WTF?

There's no truck here to check, but I'm pretty sure I got it right. That was one of my original fears.

    Indeed.
The Mrs. picked up the truck Wednesday at 5:30. They had just finished the work and I was out of town. Friday, in an act of faith, we drove the truck across New England (just got back late last night).
    The truck seems fine.
At least the work they've done seems repaired and all is good. UCA and ball joints are in correctly, tires are not canted out in a disneyesque way, lower control arm bushings replaced.

I still have a mysterious little shake that never, ever goes away despite new tires, new suspension, new carrier bearing, etc. I'd still like to track down why my truck does this, but all in all I'm happy with the truck and thankful for all the tacomaworld help.

For posterity I updated this image, it still shows a backwards installation of the ball joint, but I changed the labels to be accurate:
