bad u-joints, do you have a lift of some sort? could need a axle shim or carrier bearing drop.

I installed air bags on rear suspension to help with rear-end sag

It might be the tire balance. Did you make sure they did a dynamic instead of just a static?

Did you go to a mud terrain by chance?

My truck had the same problem and it was a u joint . It only happens when you press the gas not when it's just rolling. Right???

Really pronounced at take-off and while turning, and while driving 45 mph or faster

Try doing a tire balance first as thats the easiest to go about, then I would try a u-joint.

I heard that it could be anything from the drive line center bearing to changing differential gear oil to drive line out of balance to possible tranny problems

it can be a whole list on things, start with the things that are the easiest to fix so you can get those off the list and move on to the more expensive possible fixes.

You can remove your driveshaft and inspect your u joints. I did and that's how I found the problem. It's not that hard to do. Took me about 20 min.
