My advice is change the plugs, clean the MAF Sensor and throttle body and replace the air filter with an OEM type filer for now. Then you'll know what has been done and if it persists it'll tell you what is not causing the problem. IMO

I finally was able to do some of these recommendations. I changed the plugs (actually still had the original Denso and NGK plugs), the PCV, and cleaned the MAF, air filter, and throttle body. I don't have the original air filter assy.

The PCV looked pretty well used...

So far the engine seems to idle better but time will tell. I'll keep this post updated as I know more.

The surge when stopped is the AC compressor kicking in. Toyota seems to be a little over zealous on the idle speed compensation. If you have defrost selected, even when you are on heat, The compressor can cycle.
As far as the rough idle, I'm noticing my 2010 getting a little rougher, not real bad, and not a random shake. Almost like you can feel every cylinder fire. I'll be keeping an eye on it.

Glad to hear what you did helped. As for the plugs, they needed changed. When the electrode burns away leaving it round, it's time for a change. I painted a red line pointing at the electrode, you can see it if you blow up the picture..
