At tranny shift problem

I have a 13dbl cab with auto transmission and lately it has not been shifing smooth into 5th gear. If I slowly accelerate up to 60, it will normally shift fine and I haft to accelerate like a grandma. When I drive normally up to 60 the truck will get up to 60 but then the truck sits at high rpms for probably 15-30 seconds before it finally shifts up. I tow with my truck but only tow heavy a hand full of times a year. My truck only has 28,000 miles. I'm wondering if this is possibly a simple fix or if it is more serious.
