Cost(s) depends on the type of update plan purchased. Garmin has different choices..like lifetime updates or one-time updates etc. Frequency depends on Garmin...whenever they come out with an update....couple times a year from my experience.

Here is the update cost: http://www8.garmin.com/updatemaps/

I just updated all of the GPS maps this morning (U.S. & Canada 2012.40).

^ I can vouch for this GPS as I have this EXACT same model, the 255W. It works GREAT, very easy to use I've never even opened the manual, screen is very bright and is non-glare, overall a very nice GPS.

I have the same model and was thinking of selling it so I could upgrade to one with Lifetime map updates. Mine however has not been updated. I bought it back in fall of 2009 and had one free update in 2010. The maps are outdated and I occasionally get a message at start up stating so. I was going to do the lifetime update subscription but at $98 after taxes I'd rather just sell this one cheap and upgrade to a newer one. I like the 2450LMT that has junction view. It's selling for about $139 with lifetime traffic and map updates.

Although my town is very small and doesn't require traffic updates it is very useful when I travel out of town to places like San Diego, Phoenix or LA. I remember last summer when I went to Glendale AZ the damn GPS suggested I take a totally different route instead of I-10. I thought it was crazy and kept going to I-10. Well not even 2 miles after getting on I-10 I was in a big road construction mess. Traffic was at a complete standstill. Jill (the GPS's voice name), I'll never ignore you again, haha.

Anyways for those of you that have one of these they are very nice. I've been keeping track of my fuel consumption and mileage with this. It records everywhere you go and you can hook it up to your computer and click and drag the .gpx file into google earth and it actually traces everywhere you've been as long as you used the "Where To" feature. It also has an excel file in there for keeping track of destination time, miles, etc. Garmin is the best in the industry IMO.
