What is the downside to running the 18x9 jester in a +12mm offset? I am not understanding the offset thing. Any help is appreciated.

I would go with a -12 offset if you want to run a wider tire.

    (I have Jesters too btw)

I've got the Ballistic Havocs with a -12 offset and it works fine. The +12 is going to put your wheel closer to the UCAs and it could cause the tires to rub.

Would a 0 offset be ok? I am wanting to run a 285/75/18 without rubbing and trimming and will also be installing a 3" lift.

That said, there is little chance of getting a 285/75/18 under your truck without trimming on a 3" lift. Even a 285/75/16 usually rubs with a 3" lift and the tire you want is 2" taller.
