That's terrible. I feel really bad for this guy and his family.

I see many things wrong here...please pardon...I work in a factory where safety is a big deal. Whoever that company is.....is gonna get their ass whipped by OSHA. Assuming that video is actually REAL (and not setup).

#1 - SAFE BUTTON? Lock out tagoout?
#2 - Anytime you have a machine like that, it requires you use BOTH hands to turn it on (TWO start buttons) and those two buttons are usually on opposite ends of the machine.
#3 - The machine should have some sort of safety switches/proc sensors that it'll only apply FULL force if its in the full down position. Everything else during the downstroke, if it senses resistance - should pop back up.

It looked like some sort of screen printer machine. Weird...but the amount of force required to kill someone like that is extreme. All that *extreme* force supported on a HINGE? Unless that thing was hydraulic in nature....??

This reminds me of a guy who got killed in our plant in our pressroom. I think OSHA found some of the safety devices were defeated and the guy was doing something he shouldn't have with these device 'running'.

 

Those are good..... and very TRUE!

If anything, it teaches people to THINK and recognize potential hazards and report them. If you see problems at work and you report them and your employer doesn't do anything to fix it..... report it to OSHA or in this case, preventit.ca

 

Its def. a different world now in the work place. Where I work, if you get caught not wear the right PPE you get fined, your supervisor gets fined, and the workplace gets fined. Safety is a big deal.

 

Never assume a accident couldn't happen even if your doing everything right. We had a guy get killed while he was in a cross walk at the corner of a building because a fork lift driver wasn't paying attention when he rounded the corner. The bad part was it was the guy's last day on the job before retiring.

 

That sucks for this guy and his family, but as mentioned in a few previous posts, this seems like it could've been avoided using some common sense like powering down the machine and not putting your whole upper torso including your head in something that presses down on you while being on.

 

My crew and I responded to 2 workplace fatalities last year......It sucks and it so unnescessary.Like Neon says,be careful people.

Was it? I wouldn't blame this guy unless we know the entire story....
He's probably doing routine maintenance - something that he's done many times before.

If you watch the hinge come down, you'll notice the slider thing moves across from bottom to top. While the hinge goes back up, that slider thing moves back (from top to bottom). Assuming the 'bottom' is its HOME position.

Just prior to all this happening, it appears he hits a button or something on the front of the machine..... and the slider thing stays up at the top.

Then, he gets in there....lays down and does one thing. Gets out....gets the roll of tape....and lays in there again.

So - Do we really know the entire story?? Why didn't it crush him the first time he was laying in there? Did he press a stop safe button before getting in there? Was the safe button faulty? Did something malfunction? It coiuld be anything.
