With the 5100s the lower spring perch is flipped upside down and the spacer isn't visible. With the OME lift the spring perch is flipped to where you can easily see the spacer.

Can I run these with stock wheels? Thought I read something where there was a clearance issue, and you might need spacers? I'd be running stock steel wheels with 265/70/16

I just put this exact setup on my Access Cab with stock wheels. There is no clearance issue at all.

When they do the strut assembly it includes brand new top hats all new bushings. No shop is going to do that, for 50 they reuse your old stuff.

No for 5100s the lean spacer is a 1/4" too plate spacer.
