Would need a potentiometer with dual, possibly triple sweep, one increasing resistance the other decreasing resistance in the sweep at equal rates in the OHM range the ECM is looking for.

Possibly adding a second pedal assembly that can be locked in certain position but is hand operable could be piggy backed into the system.

I like how GM did it with the cruise control functions. That feature is (or was) actually in most GM truck ECMs just need to find the pins to energize it in the ECM connector ( they're empty locations in most trucks)
only downfall is it has to see parking brake signal to be able to activate ( this is idle up for PTO operation, which generally doesn't get used with vehicle in motion. )

 

Heck even a set pair of resistors and a switch to bump it up 500-1000 rpm. Would work across all pedal positions.

That's the problem, how do you connect the hand throttle to an electronic throttle ? And not cause other issues ?

That might work, but you would need to lock out foot feed when you do this or it sends errant signals that would confuse ECM and cause it to code. Need some parts to experiment with. I've got a couple throttle pots from fork lifts, if the ohm range is similar to foot pedal on truck it would be easy enough to make a friction adjustable remote box. Would probably still need to be a one or the other thing
