After every flight, Qantas pilots fill out a form, called a "gripesheet " , which tells mechanics about problems with the aircraft. The mechanics correct the problems, document their repairs on the form, and then pilots review the gripe sheets before the next flight.
Never let it be said that ground crews lack a sense of humor. Here are some actual maintenance complaints submitted by Qantas' pilots (marked with a P); and the solutions recorded (marked with an S) by maintenance engineers.: By the way, Qantas is the only major airline that has never had an accident.

An exploded oxygen canister caused that hole, I'd classify that as an accident. Death would be called a tragedy.

Although I don't doubt that. Investigations take forever on these things. We probably won't see a written conclusive report for several months.

I use to see the same kind of discrepancy list for military A/C. I remember reading one from a C-130 report:

    1, 2 and 4 engines leaking oil.

Signed off as, with in limits, #3 engine not leaking enough.
