The tulip bearings most likely were lost during your drive with tulip hanging out.

ZERO chance they were not in there from the factory and you DIDN'T notice. lol.

AND one of the rear bumper bolts will work in the LBJ holder in a bind.

It would never have lasted that long if it came out of the factory without them. You would have noticed IMMEDIATELY. Likely fell off when it was abused/damaged.

You put the bolts in hand tight, with red locktite, but forgot to tighten them. Why lie about it. Lol. The bent and stripped portion tells the entire story.

Not at all. It could have easily worked itself loose and that wear pattern is consistent with that. That wear pattern tells you NOTHING except that it was loose period. How it got there, we don't know.

Remember, loctite won't stick to dirty threads. If there was oil or grease in there, it wouldn't ever cure.

That said, I do think that it was probably left loose from the beginning. There's also a reason why they say to retorque everything after a few hundred miles.

No WAY the tech is checking every bolt and nut on the chassis during a simple oil change. It would take way too much time. Plus, if it was truly red loctite, he wouldn't be able to tighten or loosen it.

Torqued bolts with red loctite must work loose often for you, because they never have for me. Lol. It was never torqued.
