    Not going argue you have your training I have mine. I also have done most of my med school(taking a break because of first child) so the more knowledge the better. Agreed refresher courses are necessary and I haven't been in those combat situations since 08.

    Once again, I'm not trying to be disrespectful in anyway. My argument is strictly on an isolated GSW to a limb where significant blood is being lost. Keep in mind, studies are showing TQ application for up to 16 hours with no long-term deficits to that limb. Especially if you're looking at an extended extrication time as you stated, a TQ is a HUGE benefit if there's an arterial bleed. The golden hour is directed towards your typical multi-system trauma from an MVA, etc. Obviously we can't apply a TQ to an abdominal hemorrhage. Yes, an TQ could lead to limb loss, but if we can't get a patient to a Trauma Center or at least a Hospital in <16 hours, we're having much larger problems than a single GSW. And worst case scenario, they lost that limb at the hospital, but live to tell about it. Once again, I am not trying to be rude in anyway. The old mentality is to shy away from tourniquet application and we are trying to change that mentality. I encourage you, if you are interested enough and have the time, to read the updated studies on TQ application and it's direct correlation to preventing irreversible hypovolemic shock and the actual number of limbs lost strictly because of TQ application.

    Agree. Current standard is apply the tourniquet and leave it. Also when applying more dressing to a bleeding wound do not remove what you have on there just add more to it and apply a pressure dressing.

    I am not taking your comments as rude, and truly appreciate them. I was over generalizing the applications, as I'd hate to see someone with an ugly gash that is simply oozing, and someone say "don't worry I think I read about this in a Tacoma forum once!!" And then go on to finish out the day in pain because, hey, the bleeding stopped.

    We are singing the same tune, I am a huge fan of the tourniquet. I was part of the initial fielding of the CAT. I personally know two people that survived because of them. (Left arm amputation below the elbow, and right leg amputation above the knee, eventually loosing the left leg as well). I have also been in situations were air evac was off the table and self-ground evac was required.... with a LONG haul to a friendly trauma center.

    But it's good to know that in a more social setting, apply the tourniquet and get to the doctor ASAFP.

    Can somebody explain why it's almost always some old white male that is involved in these mass shootings? Seriously, is something going on with white males? Should we start a white man lives matter movement or something?

    Thoughts:

    I think the tourniquet discussion is complete - get some training, be prepared, use your head and do your best.

    I wonder how many people die or are killed over the weekend in a large city event like Times Square New Years eve from drugs, alcohol, domestic disputes, DUI and other 'normal' stuff

    The odds of being involved in any mass casualty event is very small

    The story now is shooter was 'going through divorce' 'had gambling debt' 'girlfriend is out of the country', etc. - who knows

    But....

    this stuff gets stopped by PROACTIVE steps, if at all...

    Manage your personal relationships
    Manage your work and finances
    Talk to people
    Seek help
    Help the less fortunate
    Keep trying to resolve disputes reasonably
    Reach out to people in crisis
    Take time with young people
    Lead by example
    Do not bully, ridicule, abuse others
    Be empathetic
    Etc.

    Strapping on your 'daily loadout rig' of ammo and trauma gear to go get more frozen burritos is not really the answer to the real problems here...

    Just my thoughts...

    I don't really care. I own many guns, I have also been a professional rescue trainer for 30+ years... you can do your thing, it really doesn't bug me, but I wish people were more aware of what the 'causes' are and less concerned with the 'reaction'...

    What are the causes of these mass shootings? The shooter usually kills himself or is killed therefore only leaving us to speculate the cause. Therefore I react by protecting myself and loved ones and being aware of my surroundings at all times.

    They aren't taking away your right anymore than you have to right to patronize an establishment that isn't gun free. Remember once you step into a hotel, it's their property and can refuse service to anyone they want. They could install x-ray machines and not even tell you.

    You should protect yourself. You have a right to. I believe the 'causes' are f'd up people... my point is merely that the only way to have less f'd up people is to consider what we can do about it and help each other beforehand

    If they install detecting device what do you believe there intent is ? It would be to not allow people in there hotel with a firearm correct.
