Same as always keep up on regular maintenance. Things that may need changed that haven't are timing belts, ball joints, shocks if original may be toast and keep an eye on your valve cover gaskets

I bought the truck with 180k so I’m trying to get back to a “base” so I can know where I am coming from. I’ve done the suspension all myself. Timing chain cover (2.7) gasket was replaced before I bought it. LBJ’s were deemed good last check up. So maybe just valve cover and spark plugs? I just did my rear diff oil too.

Since it is a 2.7 and you're near 200k I would have Toyota make sure the valve clearance is set right, they tend to tighten up on the exhaust side and if left for too long will crack the head. Had it happen to mine, ended up having to replace the head all because the previous owner didn't check and adjust every 50-75k like they recommend.

Awesome. I’ll have them do that! Anything else that’s major that I should have them do?

It's a pretty low maintenance engine and transmission really other than the valves, if it is running nicely then just keep up on the usual oil and such. Denso OEM spark plugs or NGK v-Power plugs when you do those.

This is the first I've heard of getting this done. What's the average price? Are there any signs that you could tell that they are becoming out of spec before hand or just have to take it in and find out?

Couldn't tell you the price, I had the whole head replaced and reassembled so my clearances were set when that was done and that whole process would cost much more than an adjustment. Unfortunately these engines are kind of quiet to begin with and they only get quieter as the issue gets worse (unlike old Chevies and such where you could hear the valve train making new noises and know it was time for an adjustment). Have to measure the clearance with feeler gauges record all of them, then cams have to come out and then a special tool is used to press down the bucket to remove the shim which can then be measured with a micrometer to determine what new size shim is needed after comparing all the clearances and their differences to the expected spec from the factory.

I changed my valve cover gasket over the summer decided to check the valves while in there. The exhaust valves were almost all out of spec. I didn't know it was a common issue until I started researching it. Motor has 230k on it.

Is this something that should always be done by the dealer? Or just highly recommnded?

I'd recommend it but you can certainly have someone else do it, hell you could do it yourself but the special tool and the assortment of shims isn't cheap and most Toyota shops don't have a full set to sell rather just have some to replace themselves.
