How much of a wheel spacer should I add if I already have 4.75” of bs?

That's up to you. How far do you want the wheels to stick out?
I'm in no position to tell you how to build your truck because what I think looks good may not match with what you think looks good. If I were you, I'd do a little more research on here and look at other people's builds. Looking at what other people have done and what it took for them to get there might help you decide on what you want to do on your truck. That's a more informed approach than asking a broad question where everyone will give you their opinion but it still may not be what you're looking for. It's a new member folly that we've seen time and time again. We want to be a welcoming site because there is a lot of useful information here but you'll have some homework to do as well.

    Welcome aboard and good luck.

    This and that...
This should help. Use a combination of both. New wheels or spacers will help obviously. Careful on measurements to avoid rubbing.

    I'd go to a 4 or 4.25" backspace

    For reference roughly 12.7mm=.5" , 25.4mm=1"

So for you to go to that you would need a 1/2-3/4" spacer

Offset is done in mm off the centerline of the wheel where backspace is the # of inches it is from the back of the wheel to the wheels mounting surface. So your current wheels have approx. a +6.35 offset

    Hope that makes sense

Any idea how much trimming I would have to do if I went to 4” or 4.25” bs?

No clue my tires stick out about like yours all the way around I don't know why your fronts don't seem to be. I had to mostly trim a tiny bit of the inner fender in the front and some off the inside part of the flares not really noticeable unless you are looking for it. I don't hit my cab mount unless I am really flexed at full bump and turned sharply.

Trimming tends to be a case by case scenario so space them out and check etc.

    This

Before I started or even asked any questions. I read a lot, now I have a little bit of an advantage as I was in the automotive field for 15 yrs or so as a tech. But there were certain things I wanted to know and know about products so I would research them. BTW I found using google for a certain question then having tacomaworld in the search resulted better results than the forum's search engine. I lurk and read more of people's builds product reviews etc than anything. I will give my input into stuff like this because I have a depth of knowledge with the subject.

Happens on a traditional coil lift. The fronts will get "sucked in" so to speak.

No clue Tacoma's have the same WMS to WMS front and rear so unless your wheel offset isn't the same i have no clue

So for example if I were to figure out how much more the rear was sticking out that the front would I be able to correct it with a spacer if I was able to find the thickness I needed?

Correct! But the change in width within the arc/travel of the front suspension should not be nearly as much as yours looks mine wasn't
