Defective material on the OP's specific friction plate would be my guess.

    Oh Bill...

OP, I am racking my brain but the only clutches I have ever seen that look remotely like that were ridden like a bull at the rodeo. Hard.
I also cannot think of ANYTHING that could have caused that while driving down the road. Sorry.

Oh, but the OP's is different, in that his is defective. By your logic, nothing ever made is defective, if one sample isn't defective.

Yeah I wish I would have driven the car like that. At least I will be paying for something that I used. The only thing i haven't mention was the acceleration being unresponsive as it will take over 3000 rpm to actually kick in, thats while driving it on the highway on the same day.

no it cannot be, because the Toyota Dealership wont cover the fix under warranty because they state that the clutch was defective due to wear and tear(user error)

LOL, because a Toyota clutch friction plate has never failed from defective materials.

Gonnorhea has the same clutch as all of us. Except his might have sucked from the factory.

You mean if you let out the clutch at 1000 rpm the vehicle will just sit there? If that's the case then that's why your clutch is roasted.

Oh, you can smoke a clutch without ever spinning a tire. Just put a little pressure on your clutch pedal at 80km/hr and your clutch will be gone in no time.

I can guarantee you that its not from wear and tear. if that was the case I wouldn't be bothering with this threat.

The vehicle would move fine in first gear( Clutch only) but when it came to the higher gears the acceleration was responsive ( accelerating without the clutch being pressed) .

Hm. Hard to say but at 19k miles my bet is defective but at the same time I consider it fair play for Toyota to lay down the user error card. There's no way for them to know how you treated the clutch.

I'd like to side with you and call it a defective part but I've seen first hand how some people that "know how to drive manual" drive manual so I can't say for sure. Either way most manufacturers only cover the clutch for 10,000 miles under warranty.

Absolutely agree with you but this vehicle has a history of getting its parts slipped. I will upload a video when the tired kept on locking by itself.

Talk to camvap. And possible arbitration for repair. I had to go this route on my 2016 for a unrelated issue but the process is very simple and easily winable if you have documentation service records and the like.
