    From what I understand lug-nuts were not tightened. Wheel fell the eff off.

    This almost happened to me. I took my truck to the Toyota dealer in south Austin to get the 5k service done and the lube tech left the right rear lugs loose. Luckily I had to get a tire patched and they caught those loose lugs.

    Discount did not torque the lugs on the passenger front when I got my tandem wheels installed a couple weeks ago. I tried to pull over when I heard the noise and the wheel kept going straight. Wrecked the fiberglass, rotor and whatever else Pablo finds. Discount is processing the claim and will be paying for it one way or another.
