SOS Performance makes the rear disc swap kits. It's $1150-1300 (depending on options) for the kit that uses OE 4Runner/Lexus rear calipers and rotors. If you want to step it up to 345mm rotors and 4 piston calipers, it's $2300.

I picked it up from a guy in PA, he was switching to the newer model and i got for a steal. Needs some touch up down and then can mount it.
