So, my Tacoma is ten years old and except for battery and tires, is all original. 55k miles. Garage kept.
Should I get the cooling system flushed and replaced? Are the hoses okay, or aged out?
Should I get the serpentine belt replaced?
    The dealer recommends:
    4WD service
    Spark plugs
    power steering service
    Fuel injection service
    Throttle body service
I realize the dealer is in revenue enhancement mode, but is he also giving me good advice?
The truck runs great, but I am thinking long term, and I want it to remain reliable.

If you are just a little handy with tools, you can save a cool grand doing most of this yourself. There are several threads on most of those PM's items. Pick one item at a time, research it, ask questions until your comfortable, buy the parts and tools needed and go for it. You could ask if anybody is close and would lend a hand. Once you learn, you will do a better job than 90% of the people the dealer will have do this.

With so few miles and being garage kept I don't know why they are recommending a throttle body service...throttle body gets dirty from carbon/dirt build up after being driven a lot (100k+ miles) or if you drive through a lot of dirt. My 04 had 60k miles on it when I took the throttle body apart to clean it and it was nearly spotless, and I drive through a lot of dirt.

4WD service most likely just means replacing the front diff and transfer case gear oil, super easy to do yourself if you own a wrench and some sockets.

Fuel injection service I guess I could kind of see since the truck must sit for some lengths of time, but I'm curious what they mean by "service"...are they going to take the injectors off and clean them? Or just charge you a bunch of money to run a fuel injector cleaner through the gas tank?

power steering and spark plugs are also pretty easy to DIY but more time consuming.

I wouldn't worry about flushing the coolant as long as it looks okay and you aren't having any issues with it. Hoses should be fine too unless you see any that are cracked.

As you indicated, the dealers suggestions are all about transferring $$$ from your wallet to their bank acct. Not sure what 4WD service is? They would need to provide some detail. Spark Plugs - yeah - this should be done - but anyone with common sense and some basic hand tools can accomplish this on their own. At 55K, the last 3 items on your list are nonsense, but also a possibility for the home mechanic. Lots of how-to's in the 2nd Gen forum.

PA is a salt state, correct?

In addition to the recommendations above, I’d go through it w a fine tooth comb and clean/detail everything you can. Remove and clean fasteners. Touch up every bit of rust you can. I know this is lofty but a thorough refresh will only pay dividends in the future.

For example, My 07’s tail lights were impossible to remove when I finally needed to replace a bulb. I ended up having to replace both sides because they were rusted on so badly.

It has low mileage because I have a company truck for the last 24 years so no commuting costs.
Yes PA is a salt state, and I do keep after the rust and such as well as I can. I might do this work myself, although I don’t bend as good as I used to.
Also, I am reading about the Secondary air pump filter being an issue. Where is it on a 2009 4WD Access cab?

2009 doesn’t have a secondary air pump. That’s only on newer models. I think 2011 or 12 was the first year for that

    This!!!
Both mileage and time cause fluids/oils to deteriorate and break down. Also time and environmental factors cause rubber like radiator hoses and tires to deteriorate. So change the fluids like oil, front and rear diff., radiator fluid etc. You got new tires but at 10 years get rid of the original spare tire (even if never used) and replace it with a new or newer tire. Get new radiator hoses and probably heater hoses also. I think your serpentine belt is ok but they are not expensive to replace even if you have it done (not at the dealer). Find a good competent garage to do work and maintenance on your truck not a dealer.

I did all of the work myself. I changed/replaced every fluid in the truck. Completely. Also, new spark plugs, front brake pads and rotors, and the serpentine belt. Hoses are soft and show no dry rot.
New tires and TPMS valve stems. Battery is who years old. I am ready for winter.

Hoses rot from the inside out, should have changed them. Cheap too.

You can get both the upper and lower radiator hoses for under $20 on Amazon. Gates brand and free shipping.
    Cheap insurance in my opinion.

    I have a 2009 with 130K (had 75k when I purchased back in 2015)

I've done a lot of maint. over the years:
    -spark plugs @ 90k
    -front brakes @100k
    -rear diff/transfer oil @120K
    -rear shocks@ 125K

although I've never changed the coolant, is there a DIY thread on here for that?
