Ok I did a half ass search and saw nothing...

Why does this shift knob rotate so easily, and what have you done to fix it?

I'm hesitant to dig into the issue our of concern for damaging the boot and how it connects to the knob...

any advice... it seems like it needs to locktite on the threads or whatever the heck is going on here..

Mine did too and at my last service they did something and it's tight again. Normally I do all my own work, but I figure if they'll do it for free, I'll let them :)

It is simply threaded on. Unlatch the boot (twist lock) and put another rev on the knob and reattach the boot.

I couldn't get mine to line up and be tight. If I tightened all the way, it lined up at 2 o'clock. At 12 o'clock it was always loose. The boot wouldn't hold it tight.
So, I used a hole punch (i.e. for paper) and punched some holes in a piece of a rubber gasket.
I then put the cut out rubber 'circles' inside the shift knob (I used 3). I put the shift knob back on and tightened to 12 o'clock. It stays there because the rubber takes up the slack.
NOTE: I have an automatic but it's probably the same.

Yes. ATs are the same except that the threads are M10 instead of M12 and the boot snaps on instead of being a twist lock.

The shims that you made are similar to what @7r41lbr34k3r uses to align his custom knobs:
    https://www.tacomaworld.com/threads...-2-1-4-diameter-utilizes-factory-boot.534205/

And, there is always his twist lock and snap on collars for custom knobs:

Cheap rubber crap. My 2nd gen had a leather wrapped knob. My third gen is junk.
