When I first start up when the engine is cold (overnight) I get a stumble/hesitation for approximately 30- 60 seconds when I start driving. After that the engine smooths out and no other issues. Its worse with the AC on.

I tried premium fuel and there was no difference. I just took it in for the 5000 mile service. The dealer said there are no trouble codes and couldn't find anything .

Any suggestions? This is bugging the hell out of me and it’s a little aggravating for a 30,000 truck.

My 18 stumbles a little, just did the first oil change see if that does anything. It may be the transmission but I am used to it, only when cold short stumble.

So much for keeping it aggravation free huh? I don't think it's the motor stumbling but transmission hesitation. Main reason why I no longer have the 3rd Gen anymore as it takes the transmission a second to react to pedal input. It's more noticeable in reverse. Very noticeable when you're trying to get out of the way of incoming traffic.

Unfortunately Toyota doesn't really support the 2.7 like they do the 3.5 so no TSB's for the hesitation. It's really is Toyota's tuning that's bogging it down with the shifts and acceleration.

Had the same on my 2016 and my current 2019. The transmission is very very cold natured and wants to come up to temperature or it will buck like a bronco.

My 17 2.7 SR Auto does the same, when first accelerating moderately to 50mph cold, she will cough, stumble, today even flared up and almost barked the tires, hit 3rd or 4th gear very hard. Couple miles down the road, all ok. Been this way since new, got 33k on her now. your description is the best, "Bucks like a Bronco", never exactly the same lol.
Just for my own curiosity, my oil light may flash once on cold starts, after sitting overnight, sometimes when starting cold, will go out, flash back on for a fraction of a second, the go out again, only cold. Does your 2.7 do this? Never comes back on when running.

Mine does the same.. It shifts hard and has some hesitation but it disappears within a mile or two. Been doing that since new and I’ve had my truck since 2016 and now it has 70,000 miles . This is normal, they all even shake or sputter in drive , the tail pipe will shake. This is the motor design, even the the 22re 3rz old school motors do it with automatic trans. The 2tr-fe is the modern version of the old 4 bangers.
