That’s some kind of vacuum modulator. What’s wrong with it?

Check the vacuum diagram under your hood. Might tell you what it is.

I don’t think it is. The EGR modulator on my truck has more than two vacuum ports.

Oh shoot! I think I know what this is. That could be your Fuel pressure Regulator. Looks just like mine.

Could just be that the airway is blocked to the intake plenum, when you remove the hose you are opening it to the atmosphere so it can breath. Try disconnecting the hose from the metal tube and plug it with your finger, does that do the same thing as having it connected? That will tell you that the line is blocked somewhere, most likely at the intake plenum. I would remove the hose and clean it out, then clean out the metal tube and wherever it goes into the plenum.

That is known as the "throttle opener". It should not affect idle quality, working or not. Toyota considers it part of the t-body and does not sell it separately.

It's sole purpose in life is to hold the throttle plate slightly open under light spring pressure when the engine is off. When the engine starts, manifold vacuum on the diaphragm in the round silver enclosure pulls the throttle plate closed to the normal idle position. This action allows a little more air flow for the starting process, nothing else.
With the engine running, pull the little hose and plug it with your finger. You should feel vacuum and the idle speed should pick up several hundred (or more) RPM.
If unplugging the vacuum line and not blocking it clears up a rough idle it's likely there is a problem/restriction in the idle air supply arena and opening the hose just provides some symptomatic relief. This could be several things. A sticky or dirty IAC valve may not be passing enough air and the increase from opening the small hose is enough to correct it. The throttle plate could be gummed up. The throttle position sensor could be in error. There could be a rich condition (numerous reasons).

It's going to take some digging. If you are short on diagnostic experience pull the intake tube and check the inside of the t-body for build up, clean as necessary. From there check/clean the IAC. Google/search for vids.

I think I might be having a rich condition because I haven't been able to git rid of this damn p0420 code to save my life.
