When driving my 2011 ACLB I have to have the volume at max level to hear anything with the windows up. If I am driving with the windows down, music is not an option. Should I replace the speakers first or the head unit? Everything is stock. I know there are 2 speakers in each front door. Where there speakers elsewhere in the vehicle I should know about?

Aux back up lights, Bed lights, Re-located trailer plug, Good dooby, a.k.a. jumper cable mod, Heated seats, back up camera,
If you're turning it up all the way and getting barely anything; it could be the radio.
Maybe a plug in the back of the radio wasn't locked in...?
The stock radio, mine anyways, was nothing to write home about. I swapped mine for a Kenwood single din, and i was shocked to hear the life that came to these crappy speakers.
If you know somebody else that has the Tacoma of the same vintage you can swap radios and find out if it is indeed the radio.

    Thanks.

No prob. They are very tiny and under powered. You can add a set of larger speakers to the rear doors in place of the silly little speakers. The door guts have cutouts for them, just no grills on the plastic panel. Even without the grilles, the larger speakers add better fill to the cabin.
