Lots of offroaders do this to increase clearance. No exhaust issues in the cab since it is still routed away from the cab.

Alright doll-face, c'mon and bore me!
I'm not going to say that chopping that tail pipe off is surely going to cause CO poisoning and drain bamage down the road, but allow me to mention that air flow around any vehicle doesn't always behave the way you'd think it would.

Simple example: With my last pickup, I liked to have the driver's side window open and also the rear sliding window open a bit to get some air flowing through the cab. This was fine when I had a crossover tool box in the bed. When I removed the tool box, no matter how slightly I had that rear window open, a tree leaf from the bed would occasionally come shooting up through the cab and hit the dash or windshield like it was fired from a shotgun, thus scaring the crap out of me. Even at modest speeds, like 30 to 40mph. One would think that air would flow from the side window out the rear... not true. Of course this effect was caused by air turbulence in the bed, but there is turbulence all over. Just because the end of the tail pipe is behind the cab doesn't mean that you've left all those exhaust gases behind.

    Just sayin'.

What are your thoughts if you have a shell and only have the front windows down occasionally (on an access cab)?

Even if your question was more specific, when it comes to air flow, and unless you've got some sort of sophisticated computer model, you either have to just try it or (with exhaust gases especially) follow "best practice". IMHO

Isn't the concern with cut exhaust pipes focused mainly around the fact that the cab has two huge vents on the back of it? If the gas was to travel between the bed and the cab then it could then travel into the vents and into the cab. (obviously more so when not moving than when moving)

This is more so a question, I'm curious.

I dunno'. My caution with exhaust systems comes from owning a few vehicles whose "structural integrity" had been, uhhhh, compromised by corrosion*. Yeah, the vents on our Tacos are in the back of the cab, but that's probably not the only way exhaust gas can find its way into the cab.

(* - just a fancy way of saying they were rusted to shiat underneath)
