    No accidents? Perhaps you've missed planes falling out of the sky, because technology failed. I have witnessed $50K worth of tooling get destroyed in less than a second due to a failed $50 prox sensor.

    Not being a dick, but even with out operator error, system(s) failure tend to be on the catastrophic side.
