    Yes you can do that, but that’s just finding ways to place blame not on the company too. I’m not saying it isn’t a way to look at it, but just because you look at it that way doesn’t mean there isn’t an accident involved. There’s always a reason the accident happened, doesn’t make it any less of an accident.

    Depends on what party you look at it from. Like I said you can’t twist your view or words, doesn’t mean it’s not an accident. Accidents aren’t always bad. It’s simply unplanned or unintended.

    The 5 Whys method determines root cause, which is ultimately the cause for the incident. I push supervisors here at work to use 5 Whys, to prevent the same incident from happening again. Of course we also implement Lean, Just in Time, Kaizen and 5S as well.

    Lol, I have been there and I am so glad I do not have to implement those practices anymore. It always seems it was like a flavor of the day kind of thing with those policies. Do it this way as long as it was the people in charge way of thinking and next month someone else in charge would have another great way and everyone had to change to that way of doing things. It is hard to push policies that you don't agree with, really hard.

    5 Whys is a good tool and I think Just in Time is too, to help reduce inventory in the company. 5S helps keep your place neat and organized. I’m not too much for lean or kaizen though. Japanese business models developed mainly for high volume, efficient car manufacturing processes. These two concepts don’t apply to all jobs everywhere though.

    Yes but you literally cannot say “it may be unplanned or unintended but it’s not an accident” when UNPLANNED OR UNINTENDED is literally the definition of ACCIDENT.

    I hated just in time. They would order motors based on preventative maintenance guide lines. If you had two fail in the same day you had a line down to save money on inventory? A $500 motor cost us a days production and that line clocked out and went home so the line workers lost money too, that was crazy to me. It is good maybe on ordering supplies but I don't feel it is good at all for maintenance.
