When I put my truck into reverse from going forward (for any period of time) as soon as it starts rolling backwards I'm hearing a clank sound from the front end. Also happens going from reverse to forward.

Drives fine. Nothing feels loose, I only hear it when I'm transitioning from forward to backward motion - Usually one, sometimes two clank sounds.

I *think* it might be a loose sway bar - Anyone ever had this happen in the past?

    It’s the brake pads shifting.

Check Ball Joints if not sure of condition. They can click in and out of socket.

I have 2015 and have heard it. have never found anything loose. what I can say is youd be amazed on how much movement takes place in the front suspension so I thing sometimes it gets loaded up and when you change directions its simply unloading itself. my mechanic a Toyota master has even checked it nothing loose. maybe well have to wait for a part to fall off.

Also check the lower control arm bolts/nuts. It's pretty common for alignment techs to leave them too loose causing the lca to rock bank and forth and click/clack. That's what my noise came from after a frame swap. They should be at a hundred foot pounds (pretty dang tight).

I also had this happen, it would make a clank as described and I recently had it aligned so I checked the driver side lower control arm bolts and sure enough it was loose. I tightened it down and now the sound is gone. Thanks for the information!

The clank sound is your floating brake pads. Nothing bad at all.

If the sound is the brake pads it's worth inspecting a little closer since it could be a mild issue.

I was getting a noticeable clunk after letting the brakes off when I'd start moving again. Also noticed a metallic rattle that would stop with any pressure on the brake pedal.

When I got around to inspecting it I noticed the anti-rattle spring was snapped. I ended up doing a complete brake job but replacing this $3 spring (Toyota part number is 04947-0C010) would have fixed it.

If the spring isn't broken mine only had one at the bottom of the pad. When I did my brakes I put springs at both ends of the pads, which is the way it was in my old truck. Not sure why there's was only one spring, whether that's the factory or a later mechanic.

Yup, it's not a bad idea to put the front end on jack stands roll around checking that bolts are snug and nothing (ball joints, tie rod ends, wheel bearings, control arms) is obviously loose.
