So I wrote about the impact the Texas City Disaster of 1947 had on heavy industry (refining, chemical, etc.) and, through investigating industrial disasters between 1947 and 1998, how the industry vastly ignored much of the lessons learned and remained one of the most deadly in America because, although there were many safety initiatives and protocols put into place, there remained a glaring lack of a single overarching entity to communicate a single message to so speak and enforce regulation (think of the CSB and OSHA).
