    I just drilled 3 additional holes in the 3 hole cap and using the cap as the pattern and it worked just fine. No worries that I had a 3 hole cap.

    Cant remember, I just took the bolts that came with the TRD caps to Fastenal and found some button head caps that were the same thread pitch and length.
