    His wife was there. We closed the curtains and shut the emerge doors while we worked on him. Pretty rough actually, as it was totally unexpected. He wasn't too too bad when we picked him up this morning, but there was definitely something going on... Had 10/10 chest pain 45 min before we arrived, relieved by one spray of Nitro. BP 78/50, HR 48, GCS of 15, though went to a 3 en-route to the hospital then popped back to a 15 after 20 seconds.

    He coded after being in the hospital for 2 hours.

    Pretty strange, but... it is what it is.

    Wow I've never seen someone go to a 15 that quick.

    It sucked to have the family there though, then adding that they had to be forced to leave the room and most of them were screaming didn't make anything easier. Usually I'm not in these situations being in pharmacy but I was actually taking some topical nitro to the same room. Had to grab the crash cart on the way there. During the while fiasco it felt like everything was in slow motion....it was weird.

    When you go quick with 10/10 chest pain usually is your "widow maker" or left main or a long lesion in the LAD.

    Not rare, CABG is done on all critical Left main stenosis (unless other
    co-morbidities or other reasons not to do surgery) These cases get to the ED before they die on scene.
