I bled them myself, and I'm pretty confident I did a solid bleed, but you think that's worth a look? I have years of experience bleeding, but I'm also human so anything is possible. Worth noting that I don't notice any change in performance or drag when the pulsing begins, and "pulsing" is not describing "shuddering", or pulsing in brake power; but an audible pulse only. Thanks

Sounds like a warped rotor to me. Even though you said there is decent life it's possible the rotor overheated and warped.

    Just 3 tons of fun!!!

Being a 2003, I'm pretty sure you have ABS. Early Toyota ABS does wierd shit. lol

Also, being a 2003, I'm not sure if you are aware there is a brake proportioning valve up near the back end of the gas tank. That needs to be part of the bleed.

Early ABS, with air in the line does really wierd stuff.

Some say to bleed the proportioning valve before the rear brakes, some say after... working from farthest to closest the prop valve would go after the rear brakes... I bleed it first, then bleed it again between the rear and the front brakes.

Don't misunderstand. I'm not saying you don't know how to bleed the brakes. Just wasn't sure if you did it or if you were relying on the word of the previous owner. And there are quite a few 1st Gen owners that are unaware the proportioning valve needs to be bled when bleeding the system.

Also, this may not even be the source of your problem, but it's where I would start.

My last taco did not have the proportioning valve, so no, I did not bleed it! Good to know, and definitely worth a look, of I remember to do it, I'll follow up, thanks again.

If you can find a IR thermometer, drive the truck for a bit then measure the brake temperature. If the left is sticking or dragging, it will be hotter. It also help explain the poor brake performance.

I would take it apart and inspect for correct assembly, lose bolts, pad thickness, etc.
If that sound you are hearing sounds like grinding, it is probably worn pads.

You mention bleeding. As others have said, the proportioning valve needs to be bleed and depending on how long it’s been, the ABS pump does as well. Usually, the pump is bled with a special tool that opens and closes the valves in the pump. You can damage the pump if you’re not careful. The poor man’s method is to go find a place you can activate the pump a few times by mashing the brakes hard enough to activate the ABS-that should push any air out into the brake line. Then bleed as normal. Don’t forget the proportioning valve and don’t forget order is important.
