    Anyone know what part number or brand of HID headlight bulb this is or what I can use to replace it?

    That looks like a cheap Chinese H4 HID. No reputable manufacture makes those and they are definitely not recommended. Those products cause blinding glare to oncoming drivers and are illegal.

    I'd use the opportunity to gut that garbage out of your truck. See this thread for 2nd Gen headlight upgrades.
