    It appears to be a weird his and hers bathroom. The picture in this quote is the same bathroom as the first photo that you posted. Notice how there is 2 toilets and the other side of the bathroom is pink tile? I would almost guarantee there is another door on the other side of the room.

    So I found a vent that got carpeted over in my office by the previous owner. Besides the register is there anything else I need to go ahead and cut it out? I was just gonna take a carpet knife and cut the hole to size of the duct and slide the new register in.
