You should still have some dizzy adjustment independent of the cam, the off center slots just stop you from inserting it 180° out of phase. But you usually get +/- 15° of adjustment by rotating the body within the bolts. All my Toyota's have had some dizzy adjustment. But you're right that if your cam timing was off you couldn't achieve proper dizzy adjustment either.

The early 5sfe got a timetable distributor

    hqdefault.jpg
The mid gen 5sfe didn't have any adjustment like the non vvti 1uz

The last gen 5sfe had batch fire coils

Wow that's really odd, I don't think I've seen a 100% locked out dizzy ever before. Really gotta hope everything is manufactured and machined to exact tolerances. They must rely on the knock sensing circuit to take up the slack there. I'd probably whammy in the old gen dizzy. But assuming your cam timing is good the ignition timing should be ballparked to Toyota spec. I guess depending which cam this is driven off of (int/exh) you could use an adjustable timing gear haha.

the distributor rotor has a lot of time contacting each point so (I don't know but suspect) the igniter fires the coil at the right time and the distributor just makes sure it goes the right cylinder

    I could be totally wrong

It was only done for 95 and 96 (up to 97 in the 1uz)

Yeah, well even in old ass setups they aren't on all the time. The Toyota setup has a set of reluctors on the dizzy (depending on year it is a pair or just one, older yotas use this instead of a crank sensor) and when the reluctor flys by it fires the coil. It sounds like this setup is refined enough to have more points of reference and time entirely by engine position and doesn't use the rotating part of the dizzy for anything other than making sure the spark travels to the right cyl. In which case it could be off a bit and not matter. My guess is this version would have one reluctor and a separate crank sensor, and I bet the early 5sfe has two reluctors and no crank sensor.

Cam timing I think is the problem... will pull the cover off and check

And cry if it is because I've done 300k worth of timing belts on 5sfes I've owned already and tell myself this is the last one I'll need to do every time

I mean if it doesn't have bent valves that's not a hard fix

    That'd make sense.

It's much more common than you think, no shame

Ah no shit, you looked Caucasian in all those pics of you floating around.

But that kind of upbringing sees getting diagnosed, or acknowledging having mental issues as taboo and shit. Sucks.

    Anyway, I won't deviate.
