Been having a weird issue past few weeks.

    2.7 5 speed.

Sometimes when leaving a stop in first gear the truck feels like it hesitates/slips.

After fulling releasing the clutch and going maybe 1 second later it almost feels like the rear tires spin and jumps but that's not the case.

Could this be a clutch issue?

Yes it could. But it’s not consistent?

    Miles on truck?

If you cruise in 3rd above lugging speed, and then apply full throttle, does the tach rise faster than the speed?

odd. Can you dump the clutch somewhere safe? Did you touch the shaft behind the clutch peddle? Does it run roughly?

At any time do you smell the sweet smell of burning clutch?

You say you feel a shock something catching gears in the rear or transmission .

You should be able to do a burn out if the clutch isn’t slipping. If it is slipping not going to be able to burn out and you’ll probably notice instantly. If that’s the problem.

Don't smell a clutch or anything..Throw out bearing makes a little noise.

No shock or jump just like a slip after the clutch pedal is let all the way out.

I can try. Pavement would be best or maybe try it on gravel.

I'm almost afraid the clutch is lsipping.

They make noise normally. I’d put my money on that it is starting to slip. Although normally you see it 1st on the freeway or going up a mountain from my experience.

Pavement. Uphill. Doing so will eat your clutch more though. You have a 2wd or 4x4?

    You've actually tried this?

That's a classic clutch test, rather than driveline shock stuff from a stop.

Sounds like axle wrap. Tell us about your leaf springs and rear shocks (age, oem or aftermarket, any lift, etc.).
