I told ya I'd drive home a PRO over this! 2 cylinders failed leak down test. Only 16K miles.

I'm 90% sure it some bad gas. I pulled up 2 top tier gas stations near my house. I'll drop by one of them after going to the Dealership this afternoon. Unless they find something wrong.

Isn't it possible that it is either a fuel pump or fuel rail pressure regulation problem?

The fact that it only happens in certain conditions makes me lean more to a sensor/electrical or injector because it is intermittent.
Scan tool is your friend here, AND a trained person, you need to be able to read between the lines. Experience counts.

these trucks are really sensitive to shitty gas. sometimes i fill up and the entire tank will run like complete crap. i would run an obd scanner app and keep an eye on the timing advance. bet it's retarding timing causing some issues.

Mine had similar symptoms and the same on dash lights when the fuel pump failed shortly after we got the notice and were waiting for them to figure out the recall for it.

    Update.
Dealership pulled about 6 codes. Started diagnostics. #6 cylinder failed leak down test.
How could this be with a prefect running truck 80-90% of the time? One code shows multiple cylinder misfires.

Possible. But no overheating and ran perfectly 80%+ of the time. Misfire only below 2000 rpm. Plus 0 loss of coolant.

Sticky valves ? I’m new here. Joined because I have a new Tacoma on the way. Been a while since I owned a Toyota, last was a 2012 hilux when I lived in Mexico. Are engine failures common on the 3.5’s ?

I was a Diesel mechanic in the military for 6 years. Plus I've worked in the Technical area for millions of $$ worth of German made manufacturing equipment for over 30 years.
Compression test or not. This doesn't make sense.
    Loss of Compression cylinder #6?
    With.
    No loss off power.
    No loss of coolant
    No overheating
    No unusual engine noise
    Intermittent misfire below 2000 rpms.
Truck runs smoothly 80-90% of the time.
I filled it up with ExxonMobil premium and added a bottle of water/ gas treatment right before taking it to the Dealer. Wish I'd drove it a few days before taking it in.

This is why I wondered about valves. I no experience with these engines, but have found sticking valves to cause rough low rpm. On aircraft engines we compression test with a differential pressure applied with piston held at top dead center. Air can be heard from intake or exhaust if those valves are leaking. I’m surprised there’s not a lot of issues with the intake valves of direct injection engines. Not enough heat to burn off any crud, and no fuel spray to clean it. With a
Atkinson cycle pushing air back through the intake, I would think it would be even more problematic. But I’m sure Toyota knows what they are doing. I’m a hillbilly aircraft mechanic.

I went with the 2.7 that I have personally witnessed the durability of, so I’m hoping for the best. For the most part, been out of the loop with what’s going on with cars for the last 10 years or so. But have had some rotten luck with a couple automotive choices last few years and with wife also wanting to get a new car I’ve been doing a lot of research lately. The 2.7 Tacoma was the most low tech thing I could find that has good track record. Think we’re going Subaru for her, but she hates the idea of something small.

Man that sucks, can you get a diminished value compensation based on needing engine replacement ? Doubtful this will be resolved any other way

    If it goes that far.
Can't just trade this for another Tacoma either after it's repaired. I bought a rare truck. Exclusive color year Pro. I planned on keeping.
Guess if it's a castrophic engine failure. I'll ask for a brand new engine and an extended warranty on Toyota's dime.

FWIW, I run 87 octane at the cheap stations in SoCal. On top of that I put 4000 miles per year south of the border running Pemex Magna. Mexican gas has zero ethanol or additives and my Taco runs perfect! All this to say I hope the dealer can find a solution.

They need to remove the head to see if it is a valve or cylinder cracked or other (piston/ring).
If there is a issue with #6 cylinder Toyota is already ordering a engine.
If it is a valve they will install a new complete head,

I cannot speak for the #6 crack and if it happens a lot,....... does it? Is it one of those 1 in 100 things?
As for the valve 10 ways to Sunday SOMETHING can go wrong here.

I would be curious to see if these cylinder cracks or valve seat problems are common on the PRO.
The only difference in the PRO and other V6 powertrains is the exhaust.

Last valve issue I had (3 of 12 were leaking) was because of a exhaust manifold leak that cause cold air to enter the system.
This was on a in line 6 on my Jeep. It is a different motor HOWEVER it is a common cause (exhaust imbalance of pressure / temperature) that warps the valves.

Sorry to hear its mechanical, BUT Toyota will pay for this dance.
