Never seen one, tried searching for it too. My dad was a mechanic on his days off from the FD and never seen one in his toolbox. What was it used for Mr. Rusty?

 

It's a Cash Transaction Report (CTR) required by federal law. I think it was inspired as a way to track / deter money laundering and ensure reporting for taxes.

 

I have never seen one either unless they might have come in different shapes.
