Was this motor disassembled at all? Intake removed?
    You've checked this stuff?

Motor was not disassembled to my knowledge, and I have seen no signs that would lead me to believe it has been apart ever, or at least for a very ling time. I have removed the intake to clean it. Doing so only after the swap was done and the truck was running to try to fix this issue, and was done along with cleaning/checking/changing the MAF/IAC/TPS etc.

Yes, I did check all of those specs, multiple times. Along with changing out the cap, rotor, plugs, wires, coil, and crank sensor.

Both cams are still in time. Only possibility would be if the chain jumped a tooth, but the guides look good, and the chain tensioner looks to be doing its job (chain is tight with no slack). Again I'm no expert but I feel like if the timing was off a tooth the symptons would be far worse. Remember, idle to 3k rpm is when its studdering/missing, abouve 3k rpm it runs flawlessly. I'm gona need a detailed explanation as to how thats possible before I continue down the timing road.

good grounds for computer? Has the original harness engine harness been hacked on at all? Cam and Crank sensors are touchy about electrical interference. How do the sparkplugs look now?

Grounds should all be good. They've been gone through a bunch of times. Right after the swap was finished i battled a no start for a while and I believe it was due to bad/not enough grounds.

I'm no expert on reading plugs, but according the plug read pics in the back of a Haines manual they look good. Definitly not obviously bad, light black/soot on the threaded part, slightly white ash on the tip, and electrodes are clean. All four are the same, and not wet with fuel or oil.

How about cam sensor? May not have one because it uses a pickup coil in distributer.

Without a oscilloscope to check crank signal and pickup coil outputs and seeing obd2 data live(be there to see it while running myself) I'm at a loss.

Doesn't have a cam sensor outside of the distrubutor.
