I saw zero difference either way with mine. Best mpg improvement is to just slow down and drive with a light foot.

Bed cover will not increase your fuel mileage, it’s a myth. If anything, it will make it worse. Also guys driving around with open tailgates thinking it improves airflow are wrong.:)

    Aero.

If you won't slow down you have to reduce drag.

Remove the roof rack, get skids front to back (lighter being better), etc.

You are trying to punch a big fast hold in the air, changing that is really the only place you will see a difference

Even if that 91 octane got you an extra 1/2 mile to the gallon (which it won't)..you're still spending more $$ on gas, defeats the purpose of trying to improve your MPG, which is to save $$ on gas.

Depends, by testing it can make it slightly better, for example 1.8% in one study in wind tunnel testing which I believe is more reliable. However other studies indicate soft covers may not, and others (less scientific imho) saw no gain. I would say don’t buy one for mpg but if you want and need one for other reasons go for it (I have one and love it).

I saw a 1-2mpg improvement when highway driving with my canopy. Have had similar experience with past trucks as well. However, the added weight does make the truck less efficient when driving around town so it really depends on how you use your truck.

    Fastback/kammback topper.

About the only way to deal with the sucking hole of drag that you're pulling behind you when you drive.

I already run one as I like to keep shit in the back of my truck, I have a hard cover tho so with the added weight it probably isnt helping.

The best advice I can offer comes right out of high school auto shop: DRIVE THE SPEED LIMIT in the highest gear practical, avoid jackrabbit starts, use your cruise control everywhere you can (I even use it in city where I can), avoid performance add-ons.
I have a M/T TRD Off Road with a 2" lift and a cold air intake. When I'm not using a/c I make better than 21 mpg in city. On the highway I only do about 20 mpg. I pull a 20' RV and only make 10 mpg. Additionally, I'm looking into a better quality MAF to see what I see.

I'm considering a bedcap/shell to improve aerodynamics while I pull my RV

Uneven tire wear when you fill the tires beyond the recommended cold air pressure. But it works. Your truck just rolls better :)

Aftermarket intakes will not improve MPGs, not a chance. Stock is actually a cold air intake, just like on all modern vehicles.
