    It was a misfire. This happened to me. I don't know what is causing yours, but mine was a faulty fuel injector.

    Fuel pump, or filters related isn't going to clear it's self with cycling the key.

    If you say you have no hard or pending codes, then it is likely NOT a sensor, coil, etc. issue. Bad fuel, fuel filter (as mentioned), possible fuel pump going bad. Those all make sense. If it idles fine and runs in low speeds but won't accelerate or run on the highway, that points to a fueling issue.

    The clue here is a flashing check engine light. The pcm has detected a fault that can damage the catalytic converter . It puts the vehicle in a FMEM mode (failure mode effects management). From a lifetime of diagnosing Fords I would suspect a missfire or t-body issue. Toyota strategy should be very similiar. A pending code should be found with a good scanner.

    UPDATE - I STILL NEED HELP - EXTRA INFO - Installed new Ignition Coils and Spark Plugs, truck runs better but rough idle came back... Thank you everyone for the input!

    I bought the truck new and replaced the plugs at 60k miles. I noticed 2 ignition coils were burnt because the spark plugs came loose and caused blow by. 8A19D971-6D5B-4BC2-AC23-5CE17E534A70.jpg That was the original coils next to each other back in 2020 when I changed the plugs. I had no issues so I put them back in.

    At 110k miles the truck started to rough idle when I would come to a stop and reversing to park but it just started happening and then it broke down on my way home in the video.

    I scanned for codes and got misfire codes P0300, P0301, P0303, P0305 - misifiring on one side of the engine bank and that was the same side I had installed those 2 burnt up coils.

    So I ordered new OEM ignition coils and spark plugs from a Toyota Dealership in Michigan for $530 compared to my local dealer that wanted $1,114 - and I installed them the night I got them.

    Started the truck up with a fully charged battery and it idled and ran perfect. I drove it around the block and the truck ran perfect. Parked it and drove it around the block the next day and it ran perfect.

    2 nights ago I drove it to the car wash and then Autozone with no problems until I got in the drive and started reversing.

    Once in reverse, it started to rough idle and chugging (best way I can describe) I got it in the garage and haven’t drove it yet.
    Tonight I’m going to look at the throttle body even though it didn’t look too bad when I changed the plugs.

    Is there any tips on cleaning the throttle body? I think I need to be careful not to move the plate or I could throw it out of calibration, is that right?

    also I just added sea foam in a full tank of gas and haven’t driven it more than 5 miles so I’m wondering if that could contribute to the rough idle..

    You need to have a Toyota tech diagnose it. If you think a can of sea foam is the answer, go ahead, you won't hurt anything. The truck is clearly protecting itself and the causes are many. Also, as long as the problem still exists, you could foul some of those new spark plugs driving it.

    Regarding the throttle body cleaning, I usually just turn the ignition on (engine off) and put something on the pedal to open the plate. Spray and wipe out as best possible. It may struggle starting until the intake clears of solvent, and then will idle high for awhile until it relearns idle.

    What kind of coder reader do you have? If possible, look at fuel trims first, see what the fuel system is doing. You might try moving coils around left to right, see if the misfire codes change.

    So those codes, random misfire, and misfire on cylinder 1, 3, and 5. I would presume an issue unique to the passenger side of the engine.

    This does not work in drive-by-wire systems.
    The throttle plate does not open with ignition on/engine off. Here is a proof.

    Interesting, maybe the Tacoma is different. Works on all my other throttle by wire Toyota's.

    I have a very cheap Amazon code reader - I replaced the plugs and coils and the truck ran fine for 2 days but then as I was reversing in the drievway, it started rough idling with no check engine lights. I parked it and didn't drive it until I cleaned the throttle body and MAF sensor and haven't had any issues. I opened the butterfly plate by hand carefully. Definitely more hesitant than normal to start up but I drove it about 30 miles so far and it's been perfect.

    I had multiple misfires. I replaced the ignition coils and spark plugs. Truck ran fine for 2 days until reversing in the driveway it started rough idling so I parked it and didn't drive it until the problem was fixed. Symptom seemed like a dirty throttle body or MAF sensor. Never said seafoam was the answer, I asked if it could be the problem. I cleaned the throttle body and MAF sensor and the problem is gone. Didn't need a Toyota tech, just patience and the good people on these forums.

    I thought so at first, but why would the original misfire codes only be on one side of the engine? Also now the random rough idle disappeared after cleaning the MAF sensor and throttle body!

    Truck is running great now, although it was already running great after I changed the plugs and coils up until I reversed it in the driveway and got that random misfire, had to of been the throttle body or MAF sensor...

    My throttle body wasn't too dirty but I still gave it a nice cleaning. The MAF sensor looked a little dusty, which I'm sure could have effected the performance. I've never cleaned it until now, 5 years old and 111k miles.

    The truck runs perfect now, I took it on the expressway, neighborhood roads, and driveway and had no issues, drove about 30 miles. Thanks

    Haha yes you did! I did change the coils and plugs first and then still had that rough idle come back. I honestly didn’t believe it could be the throttle body because this is what it looked like before I cleaned it!56B155ED-D0C7-4CD7-BF94-05ED593A4950.jpg Wasn’t too bad but I did get some dirt from the edges and by the hinge part..

    Throttle body is very sensitive in general, so it should be cleaned every so often

    It is crazy that a dirty MAF/ Throttle body can cause such an issue, especially while driving at speed! This could have been such a bad outcome.
