Any suggestions? It starts but it just takes a while to fire up and when it does it’s at really low rpms then it kicks up to about 850ish rpms pretty quickly. 2.7L with 275k

I'm not as familiar with the 2.7 but with other engines I've resolved similar issues with one or more of the following:
-Cleaning the MAP or MAF sensor (make sure you use proper cleaner and are careful with the sensor)
    -Cleaning the throttle body
-Cleaning/replacing the Idle Air Control Valve
    -Cleaning/replacing the Throttle Position Sensor
    -Checking for vacuum leaks

The IACV and TPS tend to be expensive so before replacing them I would recommend getting a multimeter and looking up the test parameters for them.

not sure. Would low compression cause that? I know I have low compression in cylinder 3. Truck idles and runs just fine though so I keep driving it.

I seen a car on fbmp that had this issue so i searched it up and it was due to low compression in one of the cylinders, you might have the same issue

I do. I have low compression in cylinder 3. Is that why it doesn’t like to start when it’s already warmed up? It starts just fine cold but when it’s warm it cranks a little longer and starts at really low rpm then after like 3 seconds the rpm’s kick up to normal rpm’s.

And the weird thing is my truck will do almost 65 mph up a 5% incline hill in 4th gear. I thought if you had low compression it’ll run like crap?

Remains to be seen I bought the tires and wheels the rest came along
Your injectors are leaking fouling out your plugs.

It starts cold but might take longer crank times as the fuel pump circuit is only energized when cranking or the engine is running.

It will always start when warm just takes many cranking attempts.

The low compression is not helping.

    Good luck.

injectors are brand new. Only about 6 months old. Toyota diagnosed it and said injectors were the issue so I had them replaced and it was not the issue…I was pissed to say the least. They did nothing about it.

also that would make sense as to why my fuel mileage is shit.

See what your coolant temp sensor is reading, a biased sensor can cause cause it.

Gauge sensor is separate from the coolant temp sensor that the PCM uses.
You have to look at it in scan data.

Oh so I need a scanner to plug into the obd2 port?

Yes, when the truck has been sitting overnight it should read within a few degrees of ambient air temp.
With the engine at operating temp it should be the same as engine coolant temp (typically around 180-200F).

And what if it reads different?

Either the sensor is bad or there's a problem with the wiring between the sensor and PCM.

And that will cause a misfire? When Toyota diagnosed it again after they already replaced my injectors they did a leak down test and said air was coming out of the exhaust pipe so that means I have exhaust valves that aren’t closing so maybe just needs a valve adjustment?

No probably not but it can cause it to not want to start when hot.
It's just something to check and it's free to check if you have a scan tool.
