Just a quick question regarding new Toyota headlights. Do they come pre-adjusted by chance out of the box?

"technically" they should be adjusted to OEM spec. That said, I can't recommend strongly enough to so your own aiming upon installation as every vehicle is different.

Shit bolted onto other shit, and junk.
Everything holding them are plastic and thin sheet metal. The slightest bends (1*) can make a big difference at 25 ft. They are super easy to adjust yourself just measure off 25 feet and put a piece of tape against your garage door or a building wall. Then two adjustment screws per light for vertical and horizontal.

Leave on low beam then place horizontal tape at 40” high. Place a vertical tape at center of each headlight. Adjust so the brightest part of the beam is within 4” left or right of center and roughly 2” above or below horizontal. This is only for halogen or LED. This is a little different for projectors. Check your local state and municipal requirements as they may differ and must be followed. Do the best you can to find level ground for it but I usually go back adjust based upon what I see in normal driving conditions as well.

No. They should be close to the center of the range of the vertical adjustment, but there are too many variables to account for with how it mounts for it to be properly adjusted out of the box.

Read this link, find a wall and a flat parking lot with a measuring tape, a levwl and some painters tape to mark your positions.
