So far the only thing I know for certain is the knob in the dash activated the lever. All the fuses have been replaced.

Do you have a way to read engine coolant temp, using some sort of live OBD2 reader?
Is the idle normal at ~750rpm?

A stuck open Tstat will cause the engine to take forever or be unable to get to temp. Especially in these colder months. Essentially you hook up a hose to the heater core outlet and reveserse flush everything out the input. Here's a video basically showing how it's done.

Under the hood, there are 2 small hoses that carry hot water to your heater core in dash. Is one hot but not the other ? Usually a valve that lets that hot water circulate.

Should be able to rule out thermostat if the truck warms up to ~185F, and then remains at a steady temperature, verified using the OBD2 reader.

Also as stated, depending on your year truck you could have a bad heater contorl valve

As mentioned above, feel both of the heater goes going into the firewall with the engine up to temperature. Both should be hot. If one is cold you could have a plugged heater core. To back flush it I like to pull off both hoses and pour some CLR into the hose that was cold, let it sit for 5/10 mins then put a garden hose or compressed air even through the cold hose until it blows everything out the other side. If possible, i'll also connect the hoses backwards from how they were originally and drive like that for a bit then change the coolant to get the crud that was blocking the heater core out of the system

Okay, I believe your truck has a heater valve on the inlet hose to the heater core.

Edit: if both hoses are hot have you verified that the blend doors are operational? Also, being a 98 everything is cable driven. Ensure any linkage hasn't come loose or broken.
