I'm putting a winch rack on the front of the truck and would like to replace the bolts that hold the tow hooks into the bottom of frame on either side, but can't find replacements anywhere, nor the correct part numbers for them.

They have the number 8 on the 17mm head.
    Thread Pitch: 1.25
    Diameter: 12mm
    Length: 65mm (long) 35mm (short)
    Head Height: 8mm
Shank Length: 35mm (long) 5mm (short)
    Thread Length: 30mm (both)

    Any guesses?

    Bueller? Bueller?

That looks like them. Would you mind telling me how you were able to determine the part numbers? My usual sources didn't work in this case.

Mc-Master Carr is an amazing resource. Not sure where you are, but here in San Diego, I can order something like that in the morning, and it'll be here by noon.
