I have a 22 sr 4x4 with a considerable amount of mods, and also mods that weigh quite alot. Skids, sliders, and the tent most likely weigh the most. I have been told by other off roaders and even some shops I have been to that the gears on Tacomas are pretty terrible and people say a regear often helps a lot with the driving experience. I off-road a lot, but even just going up a hill on the freeway is tough and I feel like I use a lot more gas then I should be. Any thoughts ?

If you have big tires and armor it will definitely help, but it’s expensive. And if the differentials are opened it’s best to put front and rear lockers in

Lots of folks on this forum are happy with a "tune".

Yep, try the tune first. That's a $300 fix vs $3000+ one.

All vehicles are geared for a certain range of conditions and tire sizes and tacos are no exception. If you’ve added that much weight you’re probably also in line for suspension upgrades as well so it would be a good idea to read up on that topic as well. I’d suggest reading is your highest priority since it is free and will help inform better choices. To get you started try playing with the tire size tool. It can tell you what changes in RPM and speed can be expected from different tire sizes and diff ratios.
