    I see what you’re saying. TBF I’m better about this kind of thing in a professional setting vs personal matters, but it’s still something I need to work on.

    On that note, Air France 447 is another example of indecision killing a lot of people. One of the pilots started to pull up when their instruments started reading incorrectly due to inclement weather. They never realized that they were in a stall until it was too late, and the plane literally fell out of the sky.

    That's awesome and I think you mentioned it before; helps to be all-inclusive (as much as you can be).

    Think everyone should know/learn multiple languages.

    Been learning sign on and off for the past year but it's fucking tough.

    Indecisiveness can kill in many different instances, both in the professional world and not.

    I'd chalk that up with being as bad as complacency.

    You may feel that way but I bet that you would be better off having less of a difference between the two. The plane in the Hudson (not the movie but the what actually happened) is a great case of quick thinking, weighing options and making a decision. They were able to determine in a sim after the fact that the plane could have reached an airport although barely. But the pilot weighed the risks, committed to the ditching and stuck to it and is hailed a hero for it.

    Your air France example is a bit more nuanced. Yes their was a faulty sensor but if I remember correctly there was a break down in CRM and communication between the pilot and FO and the way that an Airbus operates. The fo kept pulling in the stick when the PIC was trying to take over, the airbus, when getting conflicting inputs from both sides of the cockpit, doesn't let one over ride the other so since they failed to communicate who was in control and both trying to control the plane, neither of them actually had control. The PIC was making the right decision for correcting the situation, but the FOs actions made them mute. Again, multiple failures that stack up to the flight crashing.

    That's one that tends to kill seasoned pilots. Having the experience leads you to push the envelope further and further. I can think of two high profile ones around here, one well know bush pilot was killed last year on an off strip landing where he was pushing the edge. A few years ago a well known pilot crashed after take off in central Idaho and had failed to remove the control lock prior to flight - something that can only be summed up by
    complacency.

    Honestly wikipedia has a lot of good stuff on airplane crashes in terms of the details from investigations into root cause. A current YouTube channel blancolirio is good and pretty quick to cover incidents around the country and gives a lot of great information.

    That was part of it, and also the fact that the stall warning didn’t come on since the attitude was so extreme that the computers didn’t believe the situation was possible on the air. The stall warning actually came on when nose was pushed forward, which paradoxically made the pilots pull back again on the stick when they could’ve potentially gotten out of the stall if they’d caught it early on.
