Have noticed a slight jerk when i come to a stop. Also when i accelerate from a dead stop i can feel it jerk a little. Almost feels like there is some play somewhere. Is this u-joints need replacing?

Maybe. If you feel a vibration in the driveline while accelerating that stops when you coast that is a pretty good sign. After a while you can hear them squeaking at low speed if you roll the window down and turn off the radio and AC.

honestly start simple... fill them with grease! if its less noticeable or feel like its gone away that can be a sign that they not long for the world... or replace them and pump them full and know they are good for a while, then every other change pump some in there for paranoia.

Get under your truck and push the driveline up and down, and you will know real quick what needs replacing.

Yes a ujoint is seized, same thing happened to me. Thought it was a warped brake caliper. It was more noticeable at street/ cruising speeds.

    minor tweaks
The carrier bearing can have a similar symptom (noticeable clunk when starting off) This is just me but if I'm going into it to replace u-joints I'm doing a carrier bearing at the same time, if the carrier goes bad later on and it will, you'll be taking much of the same stuff apart again.

    Reserected from the dead.
Crawl under there and twist both sides of the joint. There should be no movement.

It's always the rear u-joint. You will feel it as you twist the joint by hand.

The carrier bearing is seldome the issue. It's more likely to have an issue with manual transmissions.
