A couple of months ago i noticed my 02 4x4 with 312K miles shaking on the highway at 60=65mph. I live in the mountains and rarely drive that fast, so it's hard to tell when it started, but it was definitely well after my last tire change/alignment. My carrier bearing had a bit of play in it and I replaced it with OEM. Still feeling the shake. I notice it more in the body than the steering wheel, as in the manual shifter shakes much more than the wheel.

It's probably worth mentioning that I did flop it on its passenger side a while back, albeit VERY slowly. It's possible there was some unusual lateral pressure put on the passenger tire that day, but It drives totally fine at all other speeds.

Any thoughts before I go spend money on an alignment?

They unevenly worn or defective? I'll do a front to back rotation and see if that helps.

alignment almost never has anything to do with a shake or vibration.

Start with the tires. Rotating F-R is a free test you can DIY and see if the feeling moves.

You can also check your trans/motor mounts with a pry bar and make sure none are broken.

My money is on a tire balance

I believe mine were unevenly worn, possibly due to bad steering rack bushings. The wear was not visible though so it was a little surprising that new tires fixed it

Update: Thanks guys, I rotated tires front to rear and it really minimized the shaking. I admit I was overdue for a rotation. I plan on getting a new set of tires for winter on some stock 4Runner wheels i picked up, I expect these balanced with new rubber will completely solve the issue.
