Can't really tell from the side other than the massive gap between the cab and the bed but you can see from the back that it's definitely sagging to one side. Almost like the dreaded taco lean but worse

 

I'd like to comment but it's real hard because I don't know the
1. condition of the rest of the frame
2. The condition of the bed mounts
3. The condition of the leaf springs
Ect.

Is start with bed removal then set the frame on stands just behind the door before it jogs up and over axle. Then measure and see where the problem is.

Dose not look to hard to correct as long as the frame is in god condition.

Everything you mentioned is all very solid, I already had the bed off when I did the welding work. After the safe-t-cap install, I sanded everything down and coated it with por 15 to keep this 25 year old frame kinda fresh. It ain't perfect but it's still very good considering its age, I've seen newer trucks in worse condition.

I appreciate your suggestion with taking the bed off again and all that, but I just don't have the time as of right now. That's why I was asking earlier if anyone knows a shop that would be able to do the work.

 

I changed my oil,put my skid plate on and rotated my tires today...but somehow I feel some what inadequate.

 

I had a feeling you knew exactly what you were doing. With the work you already put into it, if you feel you need a body shop to straighten it out, you do, because you know what you're doing. Sorry I can't help but I'm sure there's many body shops around that can fix that up. Good luck with it
