    What was the giveaway for those who have had the V6 engine mounts fail?
    While driving, vibrations in certain places; floor, steering wheel, etc, vibs at certain rpms, noises at certain rpms?

    I don’t think chasing noises is the way to nirvana. I suggest you visually inspect them. You can also have someone rev the engine while you look for excessive movement of the engine.

    In other vehicles I've had with longitudinal mounted engines and (manual) transmissions, mashing the throttle in first gear will virtually always result in the stick arcing to the right a few inches.

    Yes with the NitroGear bushing installed at the same time and the last 30k miles there has not been any vibrations.

    I am getting a specific vibration felt through the foot well and steering wheel while driving, 20mph+ in D and S2-6 feathering the throttle between 1500-1900 rpms and can slightly hear a noise.
    It does not change if I use 4WD, turn right or turn left. Above 1900rpms the vibration is not present.
    Transmission, c-case, front and rear diffs I drained and refilled with RedLine synthetic fluids. I drained and refilled the transmission with 2+ gallons with a new filter.
    Alignment two weeks ago. Toyo Open Country AT3 tires at half tread without odd wear patterns.
    76k miles now.
    I have not removed the prop shaft to clean and grease the slip yoke shaft splines.

    Fan hitting the shroud would be a give away a mount has failed.

    As @gudujarlson mentioned reving the engine but with one addition. Do it very carefully in gear. And don't do it for long. You don't want to rev in gear with the brakes applied for longer then about 30 seconds, it will raise the transmission temperature fast.

    That's if you can even do it since the Corolla floor mat throttle issue.

    Watch for excess movement in the engine, either to one side, or up on one side. Since the engine is rubber mounted, a little movement is expected. Also check the tranny mount and steady rest bearing of rear driveshaft. Tranny is easy. Just try to jack up the transfer case lightly and see if it moves easily.

    Haven't seen it with 3rd gens. Only time I've seen it on this platform is with 4th gen 4Runners with fairly high miles - the fan hits the shroud.

    Auto transmission?
    Have a helper power-brake the engine in drive and reverse while you watch to see if the engine jumps around. A few seconds in each direction won't hurt the transmission

    I had same vibrations on RWD OR with lift, i tried CB drop, but got vibration between 22-25 mph, installed 3 degree rear axle shim and its gone.
